What is the difference between HMO, PPO, and EPO plans?

Health Maintenance Organization (HMO) plans to limit your dr network by restricting whom you see for doctors and hospitals.  They also require you to chose a primary care physician (PCP) whom you will need to see to get a referral to a specialist. If you do go outside of the network, you are responsible for the full payment of service

Preferred Provider Organization(PPO) plans give you the ability to go outside of your network and still receive benefits for service.  They also allow you to see a specialist without getting a referral from your (PCP).

Exclusive Provider Organization(EPO) plans you are required to stay exclusively in network and do not offer any benefits outside of your network
